Aggregate safety programs are built around haul roads, conveyors and highwalls. Those are the hazards that kill people in the numbers everyone remembers, and they get the toolbox talks. Slips and falls get a line in the handbook.
Two of them in a row have now killed miners.
What the Record Says
MSHA’s fatality report for August 13, 2026 lists an accident at 12:42 p.m. at the Perry mine in Senatobia, Mississippi, in Tate County. Mine type is surface; mined material is common sand; the listed mine controller is Granite Construction Inc. The accident classification is slip or fall of person.
That is the entirety of the public record. As of this writing MSHA has posted no preliminary accident report and no narrative, and has issued no news release on the incident. The agency has determined no cause and issued no citations. The victim’s name, age and employer are not public, and it is not known whether the worker was a mine employee or a contractor.
The fatality immediately preceding it in MSHA’s index occurred July 29 at the Tata Chemicals mine in Green River, Wyoming — an underground operation, and also classified slip or fall of person.
Where That Hazard Actually Lives
Two consecutive deaths in one classification is a small sample and MSHA has drawn no conclusion from it. But the classification itself points somewhere specific, and it isn’t the pit. Slip and fall at a surface operation means access ladders, elevated walkways, plant catwalks, stair towers, tank tops and equipment mounting points. It means housekeeping on a stairway and whether the handrail on the crusher deck is still where it was last year.
That ground is covered by MSHA Part 56.11 and, on the construction side, by OSHA Subpart D. It’s the same elevated-access exposure that governs plant work at sites like the Y-12 complex in Oak Ridge. It is also where inspection attention tends to be thinnest, because nothing about a catwalk looks dangerous the way a highwall does.
The Practical Read
For anyone running a sand, gravel or crushed stone operation, this is a prompt to walk the elevated access routes on the plant rather than the pit. Check stair tread condition, handrail continuity at landings, ladder cage integrity, ice and material buildup on walking surfaces, and whether the three-point-contact rule survives contact with the actual climb.
MSHA typically issues a preliminary accident report followed by a final investigation with a root cause finding. Neither has been published here. Until they are, the only defensible statement about this incident is what the six fields in the record say — and the only useful response is to look at your own elevated access before someone else does.
